What should I expect on my first visit?
As similar to your first visit with your physician you will fill out standard registration, medical history and insurance forms (please click here to download and print these forms). Next, you will undergo a detailed evaluation from the physical therapist including your history, measurements, physical testing, and assessment. Your therapist will then review the findings with you and instruct you in the specific cause of the injury and design an individual rehabilitative program specific to your condition. This evaluation will usually take 1 hour and will include an initial treatment and home exercise program/instructions.

What should I bring with me?
On your initial visit you need to bring the following:

  • Prescription from your Doctor (required if using an insurance company)

  • Insurance Card

  • Insurance Company Name, Phone Number, Claim Number (if using third party insurance, i.e. workman’s compensation or auto accident).

  • List of medications you are currently taking.

  • Comfortable clothing so that the therapist may easily access the injured site

  • Any exercises you were given previously, brace, splint, or orthotic device.

  • Schedule book (for appointment making).

Any additional reports you may have (X-ray or MRI results or images, operative report, other) are helpful but not needed.

What should I wear?
Comfortable, loose fitting clothing that does not restrict your movement and allows for access by the therapist. For upper extremity injuries such as the shoulder, we recommend tank top. For lower extremity injuries such as the knee, we recommend shorts. Sneakers or other supportive footwear (no sandals) are strongly advised.

Will my insurance cover physical therapy?
We take most insurance plans (see our Insurance section), and most insurance companies cover Physical Therapy. However, every plan varies on coverage (visits authorized, copayment, deductible, etc…). Although we coordinate with your insurance company, it is recommended that you call your insurance company to find out your individual policy's limits.

Who determines the Physical Therapy facility I go to?
Ultimately, it is your decision as to where you wish to attend Physical Therapy. Your physician may give you a list of local providers and your insurance company will have a list of providers who are contracted with them. However, you may indicate to your physician and insurance company where you want to go.

Many insurance companies provide for ‘out of network’ benefits which allow you to choose where you wish to attend. Please call our office for additional information.

Why should I attend Physical Therapy?
Just like if your car or computer had problems and you ignored them, the problems would progressively worsen over time. Your body is essentially a complex mechanical device, if you ignore your injury, it will progressively worsen and lead to additional problems thereby making the initial injury harder to treat and with poorer results.

Although medication may be helpful in relieving symptoms, often times it does not completely repair the injury. In Physical Therapy we aim to completely resolve your problem quickly and efficiently.
