As a courtesy to our patients, we submit all bills to their insurance carriers. Co-payments, co-insurances, and/or deductibles may be a part of your insurance plan and are expected at the time services are provided unless otherwise arranged.
Please note that we continue to contract with new insurance companies each month. If you do not see an insurance company listed below that you feel would be beneficial, please contact us. If we are not presently a provider, we will make every effort to join with that company.
We make every effort to verify benefits and notify patients of their responsibility at the time of treatment. Final determination of payment is made by your insurance company upon receipt of the claim.

However, insurance companies frequently merge and negotiate with other plans to provide for services they do not have in a particular region. Therefore, your particular insurance plan may not be listed above. You may call your insurance provider to find out if we are in your plan.
